Prior to accessing services through a local provider, an individual must go through an eligibility determination process. Although the state of South Carolina makes the eligibility decision, the process of gathering information and visiting the families lies with community providers. MaxAbilities of York County is one of those Intake providers.
If you or a family member believes you may have an intellectual disability, related disability, Autism, HASCI (Head and Spinal Cord Injury), please call toll-free 1-800-289-7012, or www.ddsn.sc.gov/apply. Someone will return your call or respond to your application in order to assist to see if the individual qualifies for South Carolina Department of Disabilities and Special Needs Services.
If the applicant meets the screening criteria, they will be contacted and offered a choice of Intake providers, and remember MaxAbilities is one of the choices! Here at MaxAbilities our Intake Case Manager’s primary job is to gather the information necessary for DDSN to determine if the individual is eligible for services. Here at MaxAbilities a home visit is conducted (although not required by the state of South Carolina) to meet with the family, discuss their needs, talk about the various services and gather needed information for the actual eligibility determination.
However, if your family member is 36 months of age or younger, please contact: BabyNet (DHHS) toll-free at 1-877-621-0865 to schedule a screening for your child. Once found eligible through BabyNet you will be offered a choice of Early Intervention providers for your child. Encouraging Steps, which is a MaxAbilities program, is a provider of Early Intervention up until age 6.
